10 Point risk assessment:

1. Is there a family history of any type of cancer?
2. Is there a history of lung cancer in your immediate family?
3. Do you have breathing problems such as shortness of breath, persistent cough, or chronic wheezing?
4. Do you have undiagnosed chest, shoulder, or back pain?
5. Have you ever smoked cigarettes, cigars, or pipes?
6. If so, did you start smoking before age 20?
7. Did (do) you smoke more then one pack a day?
8. Have you smoked for ten years or more?
9. Have you been exposed to second smoke (at home or in your work environment)?
10. Have you been exposed to environmental pollutants, such as asbestos, radon, and/or diesel fuels?
